Report: Ohio manhole rescue crew not cautious

MIDDLETOWN, Ohio (AP) - Officials say emergency workers failed to follow proper cautionary procedures when attempting to rescue a worker who died after falling unconscious into an Ohio manhole that was filled with nitrogen fumes.

The report released Friday by Butler County Emergency Management Agency Director Jeff Galloway says workers possibly overlooked the danger of the confined space when responding to the fall at Air Products & Chemicals in Middletown.

The report was obtained by The Journal newspaper through a public records request.

Three firefighters were hospitalized after breathing fumes while trying to rescue 31-year-old city worker Jabin Lakes on May 7.

The report says the Middletown fire department should use the matter as a learning tool. ---
